I, the undersigned hereby confirm that I completed a Online Safety Induction and a SITE SAFETY INDUCTION. Guidance was given in relation to the following matters. Click All Boxes to Confirm.

I am aware that any breach of site rules will result in a safety violation being issued to me. I understand that safety violations may lead to dismissal from site. **

SCROLL DOWN TO READ FULLY! WHILE ON SITE I AGREE TO:
1. Read the Company Safety Statement. This is a guide to safety on site. It identifies and outlines methods for controlling hazards.
2. Wear a hard hat, safety boots, high visibility vest / jacket, safety glasses and gloves (minimum cut 3) and use proper personal protective equipment where required, e.g., safety goggles, ear protection, respiratory protective equipment and safety harness when working at heights where no other protection is provided.
3. Report to the supervisor, without delay any defects in plant or equipment.
4. Direct suggestions to improve safety, health and welfare to those with responsibility on site. Safety on site is everybody’s responsibility.
5. Refrain from taking shortcuts on site, and only use designated routes and platforms.
6. Keep my workstation in a tidy manner, a tidy site is a safe site.
7. Abide by the Site Safety & Health Plan that is available for me to read.
8. Abide by all RAMS for work and ensure permits are completed where required in high risk areas i.e. hot works, ladder use, confined spaces, live electrical works, etc.
9. Ensure that all tools and equipment are kept in good condition and used properly for the purpose intended by the manufacturer. (inspect daily before use)
10. Work safely and take particular care when my work may affect others. (everyone is their own Safety Officer)
11. Report to site supervisor any person seen abusing the welfare facilities on site.
12. Not ride as a passenger on any item of plant unless a seat has been provided for the purpose.
13. Use mechanical means of lifting as provided and avoid heavy lifts on my own.
14. Not to come on site whilst under the influence of alcohol or drugs.
15. Not to play practical jokes or get involved in “Horseplay” on site.
16. Keep behind barriers, handrails, and obey all warning signs and notices.
17. Report all hazards observed on site to site management.
18. Ensure all ladders access is secured. And not to use untied ladders.
19. Never to use a scaffold unless it is “complete” and keep all platforms tidy.
20. Not to use mobile phone while operating machinery, plant or working at height.
21. Not to operate machinery unless I am competent and authorised to do so.
22. Check machinery daily before use.
23. Not to leave my place of work in an unsafe condition.
24. To follow the Emergency Procedures.
25. Have my Safe Pass Card with me.
26. Wear branded PPE only
27. That I have been made aware of the Disciplinary Procedures.
